DATA PROCESSING AND SHARING CONSENT FORM
I/We, the BUYER, hereby state that I/We am/are interested to avail of the products and services of (PROJECT: ALSDC, ALMI, ASN) (the “Seller”), a corporation duly organized and existing under the laws of the Philippines
I/We understand that for the Seller to provide its products and services to me/us, it may be necessary for the Seller to collect and process personal data, and share or disclose the same to banks and financing companies; Seller’s agents, employees, members of the Seller’s group of companies, subsidiaries, affiliates, parent companies, holding companies, and joint venture partners; insurance companies; vendors, subcontractors, third party service providers, or consultants that have been contracted by the Seller to provide legal, financial, technical, architectural, construction, and administrative services; other service providers who carry out work on behalf of the Seller; and other entities or offices to whom Seller is legally required or allowed to disclose my/our personal data.
For this purpose, I/we am/are giving my/our consent to the collection, use, storage, processing of my/our personal data, which includes sensitive personal data; and to the disclosure or sharing of my/our personal data to the abovementioned entities/persons for the following purposes: due diligence and background check, credit assessment and financing (whether in-house or third-party), insurance, customer service (before and after sales), documentation and communication, compliance with legal requirements, marketing, research, census, and other purpose related to the enumerated purposes.
My/Our consent shall be valid for a period of 10 years counted from the date of my/our last communication with the Seller, or 10 years from the date I/we cease to have any property rights over any of the Seller’s projects. However, despite the expiration or withdrawal of my/our consent, the Seller may retain, process, and share my/our personal data if such is relevant to any on-going or prospective legal proceedings or to the extent required by law.
I/We understand that, pursuant to Republic Act No. 10173 or the Data Privacy Act of 2012 and its Implementing Rules and Regulations (IRR), I/we have the right to withdraw my/our consent to data collection, processing, and sharing of my/our personal data by notifying the Seller’s Customer Care Representative through the following contact details: support@asiawideland.com AND by mailing a signed written confirmation of the withdrawal of my/our consent to Asiawide Office, San Pedro (Poblacion), Hermosa, Bataan. However, I also understand that depending on the nature and extent of the withdrawal of my/our consent, the Seller may not be able to continue to provide its products and services.
I/We was/were informed of my/our rights including the right to access, correction, and right to object as provided by Section 16, Chapter XVI of the Data Privacy Act and Section 34 of the IRR.
I/We certify that I/we have read and understood this Data Processing and Sharing Consent Form and fully understand that the use and disclosure of my/our personal data is reasonably necessary, required and authorized under Republic Act No. 10173 or the Data Privacy Act of 2012 and its Implementing Rules and Regulations.
